Before your appointment we ask that you complete and return the Client Intake Form which will be emailed to you following your booking.

After your consultation you may wish to contact your medical insurer to ask them about any coverage that you may have for your recommended vaccinations. For your convenience below are the Drug Identification Numbers (DIN) for the vaccinations we carry.
